To change your child’s name, simply complete and submit the application form below.  Payment will then be taken by credit or debit card. To see our fees click here.

To apply to change the name of a child (under the age of 16) by Deed Poll you must have parental responsibility and have consent from everyone with parental responsibility. For more details relating to this issue and the exceptions please see our page on Parental Responsibility.
